This 57-year-old post-bariatric surgery patient presented for arm contouring following a 100-pound weight loss achieved through gastric bypass surgery. Her examination revealed significant bilateral upper arm skin laxity extending from axilla to elbow, with particular redundancy along the posteromedial aspect of the arms. The excess tissue created functional limitations in clothing fit and daily activities. The surgical plan included bilateral brachioplasty utilizing an anterior bicipital groove incision placement, selected to optimize scar concealment while allowing complete access to address the tissue redundancy. This precise incision placement enables effective removal of excess skin while maintaining natural arm contours and preserving important neurovascular structures. Her post-operative results demonstrate improved arm contours with elimination of excess tissue, enhanced definition of the upper arm, and restoration of natural aesthetics with well-concealed incisions.
