This 43-year-old post-pregnancy patient presented for breast enhancement to address volume loss and mild ptosis. At 5’6″ and 120 pounds, her examination revealed borderline candidacy for mastopexy, with moderate tissue laxity but acceptable nipple position. After thorough discussion of treatment options, including the possibility of staged procedures, she elected to pursue a conservative augmentation approach before considering a future lift. The surgical plan included submuscular placement of 300cc low-profile smooth round silicone implants, selected for their modest projection and optimal tissue compatibility in the setting of compromised tissue elasticity. This conservative implant choice aimed to minimize additional stress on the breast envelope while achieving the patient’s desired volume enhancement. Her 18-month post-operative results demonstrate enhanced breast volume with stable tissue positioning, showing no progression of ptosis and maintaining a natural breast contour. This case illustrates successful management of borderline ptosis through conservative implant selection and careful surgical planning.
