This 51-year-old patient with Graves’ ophthalmopathy presented for upper eyelid rejuvenation. Her examination revealed dermatochalasis complicated by mild proptosis from underlying thyroid disease. The surgical plan included a conservative upper blepharoplasty with measured skin excision, selective orbicularis muscle modification, and minimal medial fat pad reduction, carefully tailored to accommodate her thyroid eye disease. Her post-operative results demonstrate enhanced periorbital aesthetics with improved upper eyelid definition while maintaining appropriate eyelid function.
